All students participating in school sponsored athletics must have the following 5 Items on File in the CHS Athletic Office:
1. LHSAA Physical Form
2. LHSAA Athletic Participation Form
3. LHSAA Substance Abuse/Misuse Contract
4. CHS Medical Release Form
5. Copy of Birth Certificate.
