Athletic Participation Information

All students participating in school sponsored athletics must have the following 5 Items on File in the CHS Athletic Office: 1. LHSAA Physical Form, 2. LHSAA Athletic Participation Form, 3. LHSAA Substance Abuse/Misuse Contract, 4. CHS Medical Release Form and 5. Copy of Birth Certificate.
